To Whom It May Concern:
I _____________________________ (full name), hereafter referred to as the host, agree to allow Whatley Swim LLC to provide swim instruction to the following parties ___________________________ ___________________ (names of participants) at my residence, located at ________________________ ________________________________________________________________(address, city, state, ZIP).
The host understands that there is a risk of personal injury and/or property damage in the course of instruction and, with this knowledge, agrees to assume the risk of any injury to the host and damages to the host’s property during training.
Specifically, the aforenamed host agrees to hold harmless Whatley Swim and all other individuals, organizations, sponsors, promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees and other participants connected with the event from all losses, damages, injuries, causes of actions, claims, or complaints in the event that the host or their property is damaged or injured in any way during the participation, instruction and/or performance of any exercise or during any activity associated with the event location.
The host understands that in case of injury, the only medical treatment Whatley Swim will provide is first aid. Host further agrees that it is the host’s responsibility to provide health, accident, and property damage coverage for the host and their property and agrees to pay all costs associated with medical care for the host and property damage to the host’s property.
I have read and understand this release and agreement and agree to its provisions. I am not under the influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity. I am over 18 years of age.
